The Incidents tool captures safety events — injuries, near-misses, property damage, and observations — with enough detail to investigate the cause and support regulatory reporting. Recording incidents promptly and consistently is the foundation of a defensible safety program.
Record an incident

Open Incidents
Go to Safety & Compliance → Incidents and click New Incident.

Classify the event
Choose the incident type and severity. The form adapts to capture the right details for that type.

Capture the facts
Record who, what, when, and where, with photos and witness statements. Be factual and specific.

Investigate and follow up
Document the root cause and corrective actions, and assign follow-ups so they’re tracked to completion.

I need to report this to OSHA / a regulator
Complete the incident record first — it gives you the structured facts needed for the regulatory form.
