Safety Observations

Capture safe and unsafe conditions seen in the field, assign corrective actions, and build a record that prevents incidents before they happen.

Safety Observations let anyone on site record what they see — both safebehaviors worth reinforcing and unsafe conditions that need fixing. Logging observations is leading-indicator safety: it catches hazards while they’re still cheap to fix, before they become incidents.

Log an observation

Open Safety Observations

From the Safety Portal, open Observations and click New Observation.

Classify it

Mark it safe or unsafe, choose a category, and note the location. Add a photo so the condition is unambiguous.

Assign corrective action

For an unsafe observation, assign the responsible party and a due date to correct it.

Close the loop

Once corrected, mark it resolved. Safe observations stay on the record as positive reinforcement and trend data.

Why it matters

Observation trends reveal where risk is building — a spike in a category or location is an early warning that something needs attention before someone gets hurt.

Troubleshooting

An unsafe observation needs escalation

If it reflects an actual event or injury, record it as an Incident as well, and link the two.
